What is the best diet after GLP-1 medication?

There isn't one diet that is proven to be the best for everyone after weight loss medication. The most effective eating pattern is one that provides enough protein, fibre and essential nutrients, supports muscle and metabolic health, helps you manage appetite and is realistic enough to maintain long term.
Research into nutrition after GLP-1 medications is still developing. While lower-carbohydrate approaches may be useful for some people, they are not the only option. For many people, a flexible Mediterranean-style eating pattern provides a practical foundation for long-term weight maintenance.
When it comes to the best diet after weight loss medications, the honest answer is that we don't yet have a wealth of long-term research. The limited evidence we do have points mainly towards lower-carbohydrate approaches — either traditional ketogenic diets or a hybrid model known as the Adaptive Ketogenic–Mediterranean Protocol (AKMP).
The ketogenic diet focuses on very low carbohydrate intake, while the Mediterranean diet celebrates colourful plants, olive oil, nuts, fish, legumes and wholegrains. The AKMP brings these two together, aiming to capture the metabolic benefits of ketosis alongside the heart, gut and longevity benefits of the Mediterranean way of eating.
Evidence suggests that lower-carbohydrate diets can be useful tools for fat loss and improving insulin sensitivity, particularly in people with obesity or metabolic dysfunction such as pre-diabetes, fatty liver or high cholesterol. When built properly (and properly is the super important word) — with non-starchy vegetables, healthy fats and adequate protein — they can be both effective and nutritionally balanced.
But they are not a magic bullet, and they're certainly not for everyone. The truth is that, for most people, maintaining a very low-carbohydrate diet long term is challenging — socially, practically and psychologically. Energy can dip, digestion may change, and life can start to feel unnecessarily restrictive, which for some people only fuels the cycle of dieting and binge eating.
So, when people ask me, "What's the best diet?" my answer is always the same:
The one you can maintain, that supports your health, and that you genuinely enjoy.
While I respect where the research is heading, my approach combines the evidence with what actually works in everyday life. That's why I recommend a flexible Mediterranean-style way of eating built around a handful of key nutrition pillars that support weight maintenance long after the medication has stopped.

Hydration – even mild dehydration can be mistaken for hunger and can affect energy, concentration and appetite regulation.
Eat foods that keep you fuller for longer –build meals around the satiety stack of protein, fibre, healthy fats, high-volume foods (such as soups, vegetables and fruit) and foods that require more chewing, like crunchy vegetables, apples and nuts. These all help you feel fuller for longer.
Nourish your gut bacteria – a diverse, fibre-rich diet helps support a healthy gut microbiome, which plays an important role in metabolic and overall health.
Choose foods that help cool inflammation – prioritise colourful plants, healthy fats, oily fish, herbs and spices while limiting ultra-processed foods.
Eat foods that bring you joy – because the eating pattern that lasts is the one you actually enjoy. Flexibility, not perfection, is what makes healthy habits stick.
